Job summary

RVNAhealth in Connecticut is seeking a Registered Nurse (RN) Hospice Case Manager to join our growing hospice team. You will manage a caseload of patients in Southern Fairfield County, including Norwalk, Wilton and New Canaan, coordinating care with an interdisciplinary team.

As the primary clinical coordinator, you will perform admissions, assessments, develop plans of care, provide symptom management at home or in facilities, educate families, and ensure documentation meets Medicare and agency

Qualifications

• RN license in CT or Compact RN license.
• BSN preferred.
• Minimum 2 years nursing experience.
• Hospice, palliative care, home health, oncology, ICU or other complex clinical experience preferred.
• Excellent assessment, critical thinking, and communication skills.
• Ability to work independently with interdisciplinary team.
• Experience with electronic medical records and mobile documentation.
• Current CPR certification.
• Valid driver’s license and willingness to travel in Southern Fairfield County.

Responsibilities

• Provide skilled nursing care and serve as the primary clinical coordinator for patients and families.
• Perform admissions, comprehensive assessments, and ongoing patient evaluations.
• Develop, implement, and update individualized plans of care with physicians, patients, and families.
• Monitor patient conditions and adjust care plans as needed.
• Educate patients and families about disease progression, medications, and end-of-life care.
• Coordinate care across the interdisciplinary hospice team and participate in IDG meetings.
• Maintain timely documentation meeting Medicare and agency standards.
