Job summary

Hartford HealthCare at Home in Connecticut is seeking a Hospice Registered Nurse to provide skilled nursing care to hospice clients in their homes or community settings, ensuring high-quality, outcome-based care. You will assess, plan, implement and coordinate care with an interdisciplinary team to support comfort for patients and families.

Ideal candidates hold CT licensure, an Associate’s degree or nursing diploma, and at least one year of nursing experience (home care or hospice preferred).

Qualifications

• Graduate of an Associate’s degree or nursing diploma program.
• One year nursing experience OR one year nursing experience in a specialty.
• Licensed to practice in the State of Connecticut.

Responsibilities

• Reviews referrals and initiates contact with client and family.
• Performs comprehensive nursing assessment based on referral data and clinical observation.
• Develops an interdisciplinary plan of care and documents goals and resource utilization.
• Uses Hospice levels of care appropriately and coordinates with the team.
• Directs care and supervision of LPNs and home care aides.
• Provides teaching to patients and families on signs, symptoms of death and bereavement.
