Job summary

Gentiva in Austin, Texas is seeking a compassionate Hospice RN / RN Case Manager to deliver personalized hospice and palliative care in patients' homes. You will be the primary contact for patients and families, guiding them through the hospice journey with dignity and respect.

You will collaborate with an interdisciplinary team to plan and adjust care, provide education and bereavement support, and ensure accurate documentation and compliance.

Qualifications

• Must hold a current nursing license in the state of employment.
• Minimum 1 year RN experience in hospice, home health, or related settings.
• Hospice or palliative care experience preferred.

Responsibilities

• Be the primary contact for patients and families, guiding them through the hospice journey.
• Deliver and document skilled nursing care per the Plan of Care in home or other settings.
• Perform initial and ongoing assessments and adjust care as needed.
• Coordinate care with IDG and physicians to refine plans.
• Provide education and support to patients and caregivers on end-of-life care.
• Maintain accurate clinical documentation and compliance with infection control standards.
• Participate in on-call and patient support activities as applicable.
