Job summary

Gentiva is seeking a Registered Nurse (RN) Hospice for a nursing role in Austin, Texas. The position involves delivering compassionate hospice and palliative care in patients' homes or other settings, guided by individualized plans of care.

The RN will perform assessments, coordinate with an interdisciplinary team, support families emotionally and education them on caregiving, and ensure compliant documentation and infection control.

Responsibilities

• Lead patients and families through the hospice journey with empathy and clear guidance.
• Deliver and document skilled nursing care in patients' homes or other care settings.
• Perform initial and ongoing assessments to tailor plans of care and monitor status.
• Collaborate with physicians, LPNs, CNAs, social workers, chaplains, and others to refine patient care plans.
