Job summary

Kindful Health in Austin, TX is seeking a Registered Nurse Case Manager for hospice care. The role administers nursing care for patients at home and coordinates with the Interdisciplinary Group to ensure quality, compassionate end-of-life support.

You will perform initial assessments, develop and document plans of care, educate patients and families, and supervise Hospice Aide and LPN/LVN team members. CPR is required, and a current RN license plus driver’s license are mandatory.

Qualifications

• Education: Accredited Nursing program and RN licensure.
• Licensure: Current state RN license; NLC rules compliance.
• Experience: 2+ years RN in clinical care; hospice exp preferred.
• Skills: Nursing skills and good interpersonal abilities.
• Must read/write English; Driver’s license required.

Responsibilities

• Conduct initial and ongoing comprehensive assessments at patients' homes.
• Develop, document, and revise plans of care for quality and continuity.
• Educate patients and families on disease, self-care, nutrition, and end‑of‑life care.
• Provide training to staff; coordinate with IDG/T and team.
• Determine scope/frequency of services based on acuity and needs.
• Coordinate with Hospice Aide and LPN/LVN for service delivery.
• Document progress notes and inform physicians of changes.
• Monitor cases for payer requirements; participate in QAPI programs.
• Perform on-call duties and ensure effective communication.
