Job summary

Magnolia Hospice in Arlington is seeking an experienced RN Case Manager to administer hospice care in patients' homes or facilities. You will coordinate with a multidisciplinary team to develop and implement individualized care plans, document patient progress, and provide education to families.

Frequent travel within the service area and participation in on-call rotation are required. The role requires RN licensure, CPR certification, and at least two years of RN experience (hospice preferred).

Qualifications

• RN license in good standing with CPR certification.
• Two years RN experience in a clinical setting; hospice preferred.
• Driver’s license and auto liability insurance; no OIG exclusion.
• Excellent organizational and written/verbal communication skills.

Responsibilities

• Administers medications and treatments per policy.
• Develops and implements the Plan of Care with patient and physician orders.
• Documents patient status accurately and promptly per policy.
• Establishes and maintains relationships with referring physicians and families.
• Participates in on-call rotations and coordinates with the interdisciplinary team.
