Job summary

Magnolia Hospice - Waco, a provider of compassionate in-home hospice care, seeks an RN Case Manager (full-time) to coordinate patient care across homes and facilities. You will work with interdisciplinary team, plan and monitor care, and ensure high-quality, patient-centered outcomes.

You will perform assessments, develop Plans of Care, educate patients and families, and participate in on-call rotation, traveling to patient locations as needed while maintaining regulatory compliance.

Qualifications

• RN licensure in good standing is required.
• Two years of RN hospice or clinical experience preferred.
• Current CPR certification required.

Responsibilities

• Administer hospice care and coordinate with a multi-disciplinary team.
• Develop and implement a written Plan of Care with physician orders.
• Document patient status accurately and timely per policy.
• Educate patients and families about care plans and end-of-life options.
• Establish relationships with referring physicians and families.
• Participate in on-call rotation and travel to patient locations as needed.
