This full-time professional role oversees and participates in clinical interventions and support for an assigned hospice caseload, coordinating with an interdisciplinary team to ensure compassionate, comprehensive patient care. The RN Case Manager updates the Plan of Care, monitors pain and symptoms, collaborates with physicians and community partners, and participates in on-call coverage as required to ensure

Qualifications

• Graduation from an accredited school of nursing; BSN preferred.
• Minimum of one year nursing experience in a hospital, home health, hospice, or related program.
• Licensed as a Registered Nurse (RN) in Arkansas.
• Valid Arkansas driver's license, personal automobile, and automobile insurance required.

Responsibilities

• Manages patient care after initial admission and assignment to a designated RN Case Manager.
• Assists in the admission process as requested.
• Interprets Arkansas Hospice, Inc. policies and procedures for nursing staff and others.
• Coordinates and collaborates with interdisciplinary team members for the plan of care on an ongoing basis.
• Updates the Plan of Care keeping team members current on patient status and changing needs.
• Facilitates effective written and verbal communication with patients, families and providers.
• Maintains communication with attending physician and community organizations regarding resources.
• Monitors pain and symptom control to ensure patient comfort.
• Rotates on-call duties to provide 24-hour service.
• Attends required meetings and participates in Performance Improvement activities.
