Arc Hospice & Palliative Care in Arlington, TX is seeking a Full-Time Hospice RN Case Manager to coordinate care for hospice patients. The RN Case Manager will conduct assessments, develop care plans, and collaborate with the interdisciplinary team to meet patient goals.

The role follows a Monday to Friday daytime schedule with participation in on-call rotation as needed. A valid RN license and hospice experience are required, with strong clinical judgment and communication skills valued.

Qualifications

• Graduate of an accredited nursing program (Associate degree or higher preferred).
• Current RN license in practicing state or multistate compact license.
• 1+ year hospice, palliative care, or home health experience preferred.
• CPR certification and valid driver’s license required.

Responsibilities

• Complete initial and ongoing assessments of patients’ needs.
• Coordinate with physicians for certification/recertification of terminal illness per Medicare regs.
• Serve as clinical point person for assigned patients, including advanced care planning and advocacy.
• Coordinate care with providers and support systems to ensure seamless hospice services.
• Develop and update individualized care plans with patient, family, and IDG.
• Provide skilled nursing care and document interventions and outcomes.
• Educate patients and families about disease progression and hospice philosophy.
• Supervise hospice aides and other care team members; revise aide care plans.
