Job summary

Magnolia Hospice - Austin is seeking a Registered Nurse Case Manager to provide hospice care across patient residences, skilled facilities, and assisted living settings. You will coordinate with the interdisciplinary team, create and update individualized care plans, and ensure high-quality, compassionate care without on-site supervision.

Responsibilities include administer meds per policy, document patient status, educate families, and participate in on-call rotations.

Qualifications

• Education: Graduate of an accredited nursing program.
• Experience: Two years RN experience; hospice preferred.
• License: Current RN license, CPR; driver’s license and auto liability insurance; no OIG exclusions.

Responsibilities

• Administer hospice care per plan of care; coordinate with multidisciplinary team.
• Develop and implement Plan of Care per physician orders; document nursing interventions.
• Document patient status accurately and timely; communicate patient needs and responses.
• Establish relationships with referring physicians and patient families; ensure care plan adherence.
• Participate in on-call rotation and visits as scheduled.
• Maintain confidentiality and comply with agency policies and applicable laws.
