Job summary

Magnolia Hospice in San Marcos is seeking a dedicated RN Case Manager for a full-time position. You will administer hospice care at patients' homes or facilities, coordinating with the care team to implement a patient-focused plan of care and ensuring timely documentation.

The role requires an RN license, CPR, and at least two years of RN experience (hospice preferred). Travel and on-call rotation are included as part of the duties.

Qualifications

• Graduate from an accredited school of nursing.
• RN license active in state; CPR certification required.
• Two years RN experience in a clinical setting; hospice preferred.
• Valid driver’s license and auto liability insurance; no OIG exclusion.

Responsibilities

• Administers medications and treatments per agency policy.
• Performs assessments and develops a written Plan of Care with physician orders.
• Documents patient status accurately and timely.
• Collaborates with an interdisciplinary team and families on care planning.
• Makes scheduled visits and participates in on-call rotation.
• Maintains confidentiality and adheres to healthcare laws.
