Job summary

Horizons Hospice is seeking a Registered Nurse to administer nursing care for patients in their homes, coordinating with an interdisciplinary team, patients, families, and referring agencies. The RN will oversee care plans, assess needs, and ensure quality, compassionate end-of-life support.

The role emphasizes collaboration with the IDG, education of patients and families, and maintaining comprehensive documentation to support ongoing care and coordination.

Qualifications

• Nursing skills defined as generally accepted standards of practice.
• Good interpersonal skills.
• Specialized palliation/end-of-life skills preferred.

Responsibilities

• Initial and ongoing comprehensive assessments of patient needs across physical, functional, psychosocial, and environmental domains.
• Determine eligibility for hospice care and assign appropriate level of care.
• Develop and document care plans and revise as needed for quality and continuity.
• Educate patients and families about disease, self-care, end-of-life care, nutrition and dietary needs; train staff as needed.
• Assess caregiver ability to meet patient needs and coordinate services accordingly.
• Initiate preventive and rehabilitative nursing procedures.
• Apply infection control and standard precautions in care delivery.
• Provide clinical directions to Hospice Aide and LPN/LVN for continuity of care.
• Document progress and ensure quality patient care with proper records.
• Notify physicians of changes in patient needs and outcomes; evaluate responses.
