Job summary

Gentiva Hospice is seeking an experienced RN Case Manager to lead in-person and telehealth hospice nursing within patients' homes or other living environments. You will assess, plan, implement, and document care aligned with each patient’s goals, while guiding families through end-of-life care with compassion.

The role requires a current RN license and at least one year of relevant experience; hospice or palliative care experience is preferred.

Qualifications

• Registered nurse with current license.
• Minimum one year RN experience in hospice, home health, or acute care.
• Experience in end-of-life care preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care including visit frequency.
• Coordinate care across interdisciplinary teams including physicians and other providers.
• Educate patients, families and caregivers on disease, self-care and end-of-life care.
• Communicate changes in condition to the care team and ensure timely interventions.
• Document timely clinical records and ensure regulatory compliance.
• Support continuity of care and transitions between care levels.
• Participate in on-call and patient support activities as applicable.
