Job summary

Gentiva Hospice is seeking an experienced RN Case Manager to lead in-home end-of-life care. You will perform comprehensive patient assessments, develop plans of care, educate families, and work with an interdisciplinary team to ensure dignified comfort and quality of life.

Responsibilities include ongoing assessments, documentation, on-call support, and coordinating care across physicians, nurses, social workers, chaplains, and others. A current RN license and CPR are required.

Qualifications

• Active RN license in the state of employment.
• Minimum 1 year RN experience in hospital, hospice, home health, or related.
• CPR certification is required.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home settings.
• Develop and update individualized plans of care with visit frequencies.
• Coordinate care with physicians, nurses, social workers, and other team members.
• Educate patients and families on disease processes, medications, and end-of-life care.
