Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to provide compassionate, in-home nursing care to patients facing life-limiting illness. You will work with an interdisciplinary team to develop personalized care plans and educate families on caregiving and end-of-life processes.

The role emphasizes comprehensive assessments, symptom management, and timely documentation, ensuring high-quality, patient-centered care across home-based settings.

Qualifications

• RN with licensure in state of employment.
• Hospice or palliative care experience preferred.
• Strong communication and documentation skills.

Responsibilities

• Perform comprehensive patient assessments and reassessments and educate patients, family members and caregivers as appropriate.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care including visit frequency and risk identification.
• Coordinate care across interdisciplinary teams and communicate with physicians and other providers.
• Provide patient and caregiver education on disease, self-care, medications, nutrition and end-of-life care.
• Participate in on-call and patient support activities as applicable.
