Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to join our team and lead patient-focused care in home settings across Georgia. You will manage comprehensive assessments, deliver skilled nursing interventions, and collaborate with an interdisciplinary team to implement personalized plans of care.

You will educate families, provide emotional support, and ensure accurate documentation while maintaining regulatory compliance.

Qualifications

• Active RN license in the state of employment.
• Hospice or palliative care experience preferred.
• Minimum 1 year of RN experience in acute care, hospice, or home health.

Responsibilities

• Perform comprehensive patient assessments and ongoing RN visits in patients' homes or alternative settings.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Develop and update individualized care plans with appropriate visit frequency.
• Coordinate care with interdisciplinary team members and physicians.
• Educate patients and caregivers on disease processes, medications, nutrition, and end-of-life care.
