Gentiva Hospice is seeking a dedicated RN Case Manager to provide compassionate, in-home hospice care in Atlanta and surrounding areas. You will perform assessments, develop plans of care, deliver skilled nursing, and collaborate with an interdisciplinary team to support patients and families with dignity and comfort.

Requirements include an active RN license in Georgia, CPR, and 1 year of hospice or home health experience.

Qualifications

• Must hold an active RN license in the state of employment.
• Minimum 1 year of RN experience in hospice, home health, or related field preferred.
• CPR certification required.
• Strong communication and documentation skills.

Responsabilités

• Perform comprehensive patient assessments and reassessments in home settings.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care with visit scheduling.
• Coordinate care with physicians, LPNs, CNAs, and social services.
• Educate patients and families on disease, end‑of‑life care, and medications.
• Document care chronologically and communicate changes to the team.
• Participate in on‑call and patient support activities as applicable.
