Job summary

Sage Hospice Primary & Palliative Care is seeking a compassionate RN for on-call, PRN shifts to support patients in their homes. You will deliver skilled nursing care, coordinate with the care team, and ensure smooth, person-centered care across the interdisciplinary network.

Qualified applicants hold an RN license, CPR certification, and have 2+ years of clinical nursing experience; hospice or home health experience is preferred. A driver’s license and computer skills are required.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification; proficient computer skills; Microsoft Office experience preferred.

Responsibilities

• Administer skilled nursing care for clients of all ages in their place of residence.
• Coordinate care with the interdisciplinary team, patient/family and referring agency.
• Assume responsibility for coordination of care in a hospice setting.
