Job summary

Sage Hospice Primary & Palliative Care is seeking a passionate PRN On-Call Registered Nurse (RN) to deliver compassionate hospice care in patients’ homes and residences. The role supports a multidisciplinary team, coordinates with families, and ensures continuity of care in a fast-paced environment.

Ideal candidates hold an RN license, CPR certification, and at least two years of clinical RN experience ( hospice/home health preferred); strong communication and computer skills are required.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient in computer use; Microsoft Office experience preferred.
• Strong interpersonal skills and ability to provide effective support to the patient and family.
• Demonstrate skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles

Responsibilities

• Administer skilled nursing care for clients at home or in residence; coordinate with the interdisciplinary team.
• Coordinate care with patient/family and referring agency; ensure smooth handoffs.
• Join our PRN On-Call RN rotation to deliver hospice care when needed.
