Job summary

Sage Hospice Primary & Palliative Care in Chandler, AZ is seeking an on-call Registered Nurse (PRN) to join our hospice team. You will provide skilled nursing care to patients in their homes and coordinate with the interdisciplinary team.

The ideal candidate holds an accredited nursing degree, current RN license, CPR certification, and a valid driver’s license. Hospice/home health experience is preferred; strong interpersonal skills are essential.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and valid driver's license.
• Two years’ RN experience in clinical care; hospice/home health experience preferred.
• Current CPR certification.
• Computer skills; MS Office experience preferred.
• Strong interpersonal skills and ability to support patients and families.

Responsibilities

• Administer skilled nursing care in patients' homes.
