Job summary

Sage Hospice Primary & Palliative Care in Peoria, AZ seeks a compassionate PRN On-Call Registered Nurse to join our hospice care team. You will administer skilled nursing care in patients' homes and coordinate with an interdisciplinary team to ensure seamless care.

The ideal candidate holds an RN license, CPR certification, and 2+ years in clinical nursing; hospice/home health experience is preferred. Strong communication and computer skills are essential.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient skill using a computer is required; Microsoft office suite experience is preferred.
• Strong interpersonal skills and ability to provide effective support to the patient and family.
• Demonstrate skill in nursing practice, ability to cope with family emotional stress.

Responsibilities

• Primary functions are to administer skilled nursing care for clients in their place of residence and coordinate care with the interdisciplinary team, patient/family and referring agency.
• Assume the responsibility for coordination of care and provide on-call nursing coverage as needed.
