Sage Hospice Primary & Palliative Care is seeking a compassionate PRN On-Call Registered Nurse to deliver skilled nursing care in patients' homes. You will coordinate with an interdisciplinary team to ensure seamless plans of care and responsive on-call support for families.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and valid driver’s license.
• Two years’ RN experience in a clinical care setting; hospice/home health preferred.
• Current CPR certification.
• Proficient computing; Microsoft Office experience preferred.
• Strong interpersonal skills and ability to provide effective support to patient and family.
• Ability to cope with family emotional stress and tolerance of individual lifestyles.

Responsibilities

• Administer skilled nursing care for clients in their place of residence.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
• Provide on-call nursing coverage with compassionate, timely care.
• Document and communicate care plans clearly to caregivers and team.
• Maintain accurate charting and regulatory compliance.
