Job summary

Sage Hospice Primary & Palliative Care is seeking a passionate on-call RN to join our hospice care team. The PRN on-call RN delivers skilled nursing care in patients' homes, coordinates with the interdisciplinary team, and supports families with compassion.

Qualified candidates hold an accredited nursing degree, an active RN license, and CPR certification; two years of RN experience in clinical care, preferably hospice or home health. Proficiency with computers and MS Office is preferred.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ RN experience in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient skill using a computer; Microsoft Office experience preferred.
• Strong interpersonal skills and ability to provide effective support to the patient and family.
• Demonstrate skill in nursing practice and tolerance of family emotional stress.

Responsibilities

• Administer skilled nursing care for clients in their place of residence and coordinate care with the interdisciplinary team.
• Coordinate care with patient/family and referring agency; ensure care continuity and documentation.
