Job summary

Agape Hospice Care is seeking a passionate RN on-call to deliver skilled nursing care in patients' homes. You will coordinate with an interdisciplinary team and families to ensure seamless care delivery.

Responsibilities include clinical assessment, care planning, and collaboration with referring agencies. This role requires active RN licensure, CPR certification, and strong communication skills to support patients and families in challenging times.

Qualifications

• Graduate of an accredited Associate or bachelor's degree in Nursing.
• Current state license as an RN.
• Two years' experience as an RN in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient skill using a computer; Microsoft Office experience is preferred.
• Strong interpersonal skills and ability to provide effective support to the patient and family.
• Demonstrate skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles.

Responsibilities

• Administer skilled nursing care for clients in their place of residence and coordinate care.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency; assume responsibility for coordination of care.
