Job summary

Sage Hospice Primary & Palliative Care in Peoria, AZ is seeking a PRN On-Call Registered Nurse (RN) to join our hospice care team. The RN will provide compassionate nursing care in patients' residences and coordinate with the interdisciplinary team to support families during transitions.

Applicants should have an active RN license, CPR certification, and at least two years of clinical RN experience; hospice or home health experience preferred.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.

Responsibilities

• Administer skilled nursing care for patients in their homes.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
• Assume responsibility for coordination of care and documentation.
