Job summary

Elara Caring is seeking a Hospice Certified Nurse Assistant/Hospice Aide to join our home health team. You will verify plans of care, provide care under RN direction, document nursing notes, and communicate changes to the RN Case Manager and physician.

Qualifications

• High School Diploma or GED is required.
• 1 year of Hospice Aide or Nursing Assistant experience required.
• State Hospice Aide training program and competency evaluation required.
• Experience in a home health environment preferred.
• 50% travel required.
• Valid driver’s license, insurance and reliable transportation.

Responsibilities

• Verifies the Plan of Care before each visit and provides care per RN delegation and physician orders.
• Delivers nursing visits and documents care in skilled nursing notes.
• Communicates changes in condition to the RN Case Manager and physician.
• Reports unsafe conditions to the RN Case Manager and physician as appropriate.
• Implements patient safety plans using patient, family and community resources.
• Assists patient in learning self-care techniques while delivering quality care.
