Job summary

Elara Caring is seeking a Hospice C.N.A /H.H.A. to join our team providing compassionate care in patients' homes. You will verify plans of care, document medications, and support RN case management while delivering high-quality, patient-centered care.

Ideal candidates have at least 1 year in hospice or nursing assistance, a state hospice aide training, and the ability to travel up to 50% with reliable transportation.

Qualifications

• High School Diploma or GED required.
• Minimum 1 year of hospice aide or nursing assistant experience.
• Completion of a state-established hospice aide training program.
• Home health experience preferred.
• Ability to travel up to 50% of the time.
• Must be able to lift up to 50 pounds and assist with transfers.
• Valid driver's license and reliable transportation.

Responsibilities

• Verify the Plan of Care prior to each visit and follow delegation by the RN.
• Document medication regimen and update medication profile.
• Provide compassionate, quality patient care and education.
• Collaborate with the RN Case Manager during normal business hours.
• Implement all available actions to prevent avoidable hospitalizations and ER visits.
