Affinity Health Management seeks a Registered Nurse to plan, organize, and direct home-based hospice care in the Philadelphia area. The nurse will assess patient needs, develop care plans, administer medications, educate families, and coordinate with the interdisciplinary team.

Emphasis on community health, comfort-focused care, and collaboration with families in patients' homes, with on-call duties and ongoing professional development opportunities.

Qualifikationen

• Graduate of an accredited school of nursing.
• Licensed Registered Nurse in good standing.
• Experience as a professional nurse within the last three years (1 year) or a BSN with two years’ experience in public health/home care/hospice preferred.
• Supervision of hospice aides is not required.
• Background checks required by regulation.
• Excellent observation, verbal and written communication skills; basic math and nursing competencies.

Aufgaben

• Assess patient and family to determine hospice needs and plan of care.
• Provide professional nursing care per the plan; administer medications.
• Develop and revise the plan of care as patient status changes.
• Initiate and revise plans of care and assess patient status regularly.
• Identify nursing diagnoses from health assessment data and implement actions.
• Educate patient and family on nursing and care needs; promote self-care.
• Maintain accurate clinical notes and communicate with physicians.
• Coordinate care with the interdisciplinary team; participate in on-call duties.
• Supervise ancillary personnel when required and ensure equipment and supplies.
• Attend interdisciplinary group meetings; ensure safe, compassionate patient care.
