Job summary

Affinity Health Management in New York seeks a Registered Nurse to plan, direct and coordinate hospice care for patients in home and community settings. The RN will assess needs, develop care plans, administer medications, and provide education to patients and families.

You will work with an interdisciplinary team, participate in on-call coverage, and ensure high-quality, compassionate care while complying with licensing and safety standards.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse licensed to practice.
• 1 year of nursing experience in last 3 years or BSN with 2+ years in public health/home care/hospice preferred.
• No mandatory management experience; may supervise hospice aides.
• Offer contingent upon references and criminal background checks.
• Excellent observation, verbal and written communication, problem solving, and basic math skills.
• Ability to walk/stand for long periods; lift, position and transfer patients.
• Licensed driver with insured vehicle in good order.
• CPR certification required (hands-on demonstration).

Responsibilities

• Complete initial and ongoing patient/family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Write and initiate plan of care; re-evaluate as patient needs change.
• Initiate and revise plan of care as patient status evolves.
• Use health data to determine nursing diagnoses.
• Develop care plans with patient/family involvement; include palliative actions.
• Administer medications/treatments as prescribed; provide education.
• Counsel patient and family on nursing needs and self-care techniques.
• Assist with activities of daily living and support self-sufficiency.
• Act as Case Manager for assigned caseload; maintain notes and coordinate care.
