Qualifications

• Graduate of National League for Nursing-accredited nursing program.
• Current RN licensure in Ohio.
• 1 year of professional nursing experience within the last 3 years, or 2 years with a BSN and at least 1 year in public health, home care, or hospice nursing preferred.

Responsibilities

• Performs initial and ongoing comprehensive patient and family assessments to determine hospice needs.
• Delivers professional nursing care using the nursing process and plans of care.
• Writes and revises care plans as patient status or needs change.
• Administers medications and treatments as prescribed; provides patient/family education.
• Coordinates with physician and interdisciplinary team to ensure continuity of care.
• Participates in on-call duties and maintenance of equipment and supplies.
