Job summary

Affinity Health Management in Indianapolis, IN seeks a registered nurse to plan, organize and direct hospice care in home and community settings. The nurse will build on community resources to meet needs of patients and families within their homes and communities.

The nurse will complete comprehensive assessments, develop and update care plans, administer prescribed treatments, coordinate with physicians and the interdisciplinary team, and provide health teaching to patients and families.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse licensed in good standing.
• One year of professional nursing experience in the last three years preferred; public health/home care/hospice experience preferred.
• Excellent observation, communication and problem solving skills; nursing skills per competency checklist.
• Must have CPR certification and a valid driver’s license.

Responsibilities

• Complete initial and ongoing assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise care plans with patient and family based on nursing diagnosis.
• Initiate care plans, revise as patient needs change, and coordinate with the physician.
• Maintain accurate clinical notes and communicate with physicians and the interdisciplinary team.
• Teach patients and families about care, medications, and self-care techniques.
• Participate in on-call duties and ensure equipment and services are available.
