Job summary

BJC Home Care in Illinois is seeking a Hospice - Registered Nurse Case Manager to support patients and their families. The role operates Monday–Friday, 8:00am–4:30pm, with on-call and holiday rotations, offering relocation options and a sign-on bonus to eligible candidates.

The position emphasizes evaluating patients, coordinating care through an interprofessional team, and delivering compassionate, family-centered care in home or SNF settings.

Qualifications

• Nursing Diploma or Associate's degree in nursing.
• RN license with valid driver's license.

Responsibilities

• Assess patient preferences and barriers to involvement in care and develop care plans with the interprofessional team.
• Coordinate all aspects of care across the continuum and communicate plan of care across care continuum.
• Promote equity, empathy, and patient-family-centered care; advocate for patient families.
• Evaluate patient condition and update plan of care in EMR; ensure evidence-based practice.
