Job summary

BJC Home Care in St. Louis is seeking a Hospice Registered Nurse Case Manager to support patients and families in end-of-life care. The role follows a Monday through Friday schedule from 8:00am to 4:30pm, with weekend and on-call rotations to ensure continuous care across regions.

The ideal candidate will be an RN with hospice/home health experience, valid driver's license, and a commitment to compassionate, patient-centered care in the community setting.

Qualifications

• RN licensure and valid driver's license required.
• Experience in hospice or home health care preferred.
• Strong communication and interdisciplinary teamwork.
• On-call coverage and weekend rotation required.

Responsibilities

• Assess patient needs and coordinate care with families and clinicians.
• Develop and document care plans with measurable goals.
• Provide symptom management and bereavement support.
• Ensure care continuity across facilities and EMR updates.
• Participate in on-call coverage as part of the team.
