BJC Home Care in St. Louis is seeking an experienced Hospice - Registered Nurse Case Manager to support patients and families in end-of-life care. Work pattern is Monday to Friday, 8:00am to 4:30pm with weekend and on-call rotation.

Qualifications

• RN with valid license and nursing education.
• On-call and weekend coverage may be required.
• Experience levels: 0-2 years or 2-5 years preferred.

Responsibilities

• Assess patient needs and plan of care in collaboration with the interprofessional team and family.
• Coordinate all aspects of care and ensure communication across the continuum of care.
• Advocate for patient and family, supporting emotional, cultural, and spiritual needs.
• Evaluate changes in patient condition and update care plans in EMR.
