Job summary

BJC Home Care is seeking a Registered Nurse Case Manager to support hospice patients and families in the St. Louis area. This role follows a Monday–Friday schedule from 8:00 am to 4:30 pm with weekend and on-call rotation, and includes coverage across St.

Louis regions. The RN will evaluate patient needs, coordinate a multidisciplinary plan of care, and ensure high-quality symptom management and compassionate care in home or facility settings.

Qualifications

• Nursing diploma or associate's degree required with RN license.
• Bachelor's degree in nursing/home health preferred.

Responsibilities

• Assess patient preferences and barriers to involvement in care.
• Develop, implement and document individual care plans with defined goals.
• Promote equity and empathy in interactions with diverse populations.
• Evaluate changes in patient condition and update the plan in EMR.
• Work is safety-sensitive and requires alertness.
