Job summary

BJC Home Care is seeking a Hospice - Registered Nurse Case Manager to provide end-of-life care and support to patients and families. This on-site role operates Monday through Friday 8:00am–4:30pm with on-call and holiday rotations, serving the Illinois service area.

The team emphasizes compassionate, multidisciplinary care and professional development through the BJC RN Career Ladder. A valid nursing license and state registration are required.

Qualifications

• Nursing diploma or associate's degree in nursing.

Responsibilities

• Assess patient preferences and barriers to involvement in care, developing plans with defined goals with the interprofessional team and family.
• Proactively plans and communicates the plan of care across the continuum of care and EMR.
• Evaluate changes in patient condition, update plan of care, and ensure evidence-based practice.
