Job summary

Qualifications

• Current unencumbered LPN license for Ohio.
• One year acute care experience required.
• Experience in Medicare-certified home health preferred.

Responsibilities

• Care management and service delivery in home settings; develop and implement care plans.
• Ensure compliance with standards, regulations, and agency protocols.
• Communicate with care team and document care activities; provide caregiver education.
