Ohio Living is seeking a Palliative Medicine Nurse Practitioner to provide patient care across home health, hospice, and clinic visits in compliance with regulatory standards.

The role emphasizes building trusting relationships, accurate documentation in EMR, and collaborative care with the medical team. Requires Ohio NP license and two years of advanced practice experience, with geriatrics experience preferred.

Qualifications

• Current unencumbered license for the state of Ohio to practice as a Certified Nurse Practitioner or Physician Assistant required.
• Holds narcotics and controlled substances licenses, permit numbers, and provider number for Medicare and Ohio Medicaid.
• Two years of experience as an advanced practitioner required.
• Experience in geriatrics preferred.
• Knowledge of evaluation and management coding.
• Experience in home health, hospice, physician office, or long-term care settings preferred.
• Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint) and Internet.

Responsibilities

• Care and Relationship Management: interpret symptoms, obtain histories, perform exams, prescribe and provide instructions.
• Collaborate with physicians, PAs, and nurses to form a high performing medical team.
• Maintain comprehensive EMR documentation and ensure Medicare-compliant records.
• Assist in program development, education, and policy updates to meet regulatory requirements.
• Perform duties in home health, hospice, clinic or facility settings as needed.
