Job summary

Ohio Living is seeking an Advanced Practitioner to provide patient care during home, facility, and/or clinic visits in alignment with organizational and regulatory standards. The role emphasizes diagnostic evaluation, treatment planning, and collaboration with physicians, nurse practitioners, and staff to deliver high-quality care.

The candidate must hold a current Ohio license as a NP or PA, have at least two years of advanced practice experience, and ideally familiarity with geriatrics.

Qualifications

• Current unencumbered license for the state of Ohio to practice as a Certified Nurse Practitioner or Physician Assistant.
• Two years of experience as an advanced practitioner required.
• Experience in geriatrics preferred.
• Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required.

Responsibilities

• Care and Relationship Management: interpret symptoms, diagnose conditions, obtain histories, perform exams, prescribe and manage care, document in EMR, collaborate with the care team, ensure compliance with Medicare standards.
• Operations Management: assist in development and management of educational programs.
• Quality & Compliance Management: assist with policies and procedures, protect patient confidentiality, perform other duties as assigned.
