Job summary

Reserve Care, formerly Hospice of the Western Reserve, delivers hospice, palliative and grief support across Ohio. The Nurse Practitioner role provides independent patient care within the Ohio scope and contributes to standards of practice and team-based care for patients and families at life’s important moments.

We seek a licensed NP with an Ohio license and a Master of Nursing degree, capable of leadership and collaboration with RC staff and healthcare providers to advance compassionate

Qualifications

• Maintains a current Nurse Practitioner licensure in state of Ohio.
• Maintains a current Certificate of Authority in state of Ohio.
• Master of Nursing Degree.
• Minimum of two years in clinical setting; ability to communicate well.

Responsibilities

• Provides independent direct nursing care to patients, within the Nurse Practitioner role as delineated in State of Ohio.
• Participates in development and implementation of new standards of practice.
• Maintains and promptly completes all required records and reports.
• Participates in Transdisciplinary team.
• Serves as a consultant to staff caring for patients in long-term care settings and home settings.
• Assists in identifying learning needs of staff through formal and informal assessments.
• Maintains knowledge of current end-of-life care literature and shares with staff.
