Ohio Living seeks a compassionate RN to join our hospice field team, delivering in-home care to patients across Ohio. You will assess needs, plan and implement care, manage symptoms, and coordinate with health professionals to ensure high-quality hospice services.

The role requires an Ohio RN license, at least 1 year in acute/medical-surgical settings, and willingness to work weekends, evenings, and holidays with on-call duties. EMR experience and a valid driver's license are preferred.

Qualifikationen

• Current unencumbered Ohio RN license.
• At least 1 year of experience in acute care or medical-surgical nursing.
• Experience in Medicare-certified home health preferred.
• Proficiency with Windows, Microsoft Office, and the internet.
• EMR experience preferred.
• Must read, write, speak English.
• Valid driver’s license and insurable under carrier policies.

Aufgaben

• Provide ongoing assessment and nursing care to patients in homes and health care facilities.
• Develop and implement a comprehensive care plan addressing current needs.
• Manage pain and symptom control for patient comfort.
• Coordinate with other health care providers about changes in condition and patient needs.
• Educate patients and caregivers as necessary.
• Prepare for IDG by reviewing Care Plan and medications as applicable.
• Audit and maintain patient records in accordance with policies and regulations.
