Job summary

Blue Summit Hospice and Palliative Care seeks an LPN/LVN to administer nursing care to terminally ill hospice patients under physician orders and the IDG plan of care. The role requires adherence to policies, collaboration with the RN, and education of patients and families.

The LPN/LVN will participate in plan implementation, document care, and may travel to patient homes to provide services as scheduled.

Qualifications

• Graduate of a state approved practical nursing program.
• Currently licensed to practice as an LPN/LVN in the applicable state.
• Minimum of one year nursing experience preferred.
• Ability to exercise initiative and independent judgment.
• Completion of a Hospice training program prior to providing care.

Responsibilities

• Adheres to established policies and procedures.
• Implements the IDG nursing care plan for each patient.
• Provides nursing services, treatments and diagnostic procedures as assigned.
• Observes signs and reports changes to the physician and RN.
• Educates patient and family on care needs and self-care techniques.
• Maintains complete records of observations and care.
