Job summary

Compassus is seeking an LPN/LVN to provide holistic hospice nursing care in Washington state, under the supervision of a Registered Nurse. You will observe patients, implement the Plan of Care, educate families, and document observations in the electronic health record.

Qualified candidates will hold a Washington LPN license with at least 1 year in acute care; home health experience is preferred. Strong communication and organization skills are essential to collaborate with a multidisciplinary

Qualifications

• Washington state requirements for LPN/LVN in home hospice care.
• Minimum 1 year of acute care experience; home health preferred.
• Current Washington LPN license upon hire and CPR/BLS certification.

Responsibilities

• Provide LPN/LVN hospice nursing services under RN supervision.
• Observe and report patient data to the RN for Plan of Care.
• Educate patients and families about care needs and changes.
• Document observations accurately and timely in the EHR.
• Triage and support the care team as needed.
• Adhere to hospice policies and regulatory guidelines.
