Job summary

Blue-Summit-Hospice-and-Palliative-Car is seeking a licensed practical/vocational nurse (LPN/LVN) for hospice care in Roswell, GA. The role is full-time with travel to patient homes and on-site hospice office duties.

The LPN/LVN administers care under physician orders and IDG plans, with responsibilities including teaching families, maintaining records, and participating in IDG conferences. A hospice training background is preferred and on-call duties are required.

Qualifications

• Graduate of a state approved school of practical (vocational) nursing and currently licensed in the state(s) in which practicing.
• Minimum of one (1) year experience in nursing, preferred.
• Acceptance of philosophy and goals of this Hospice.
• Ability to exercise initiative and independent judgment.
• Completion of a Hospice training program prior to providing care.

Responsibilities

• Understands and adheres to established policies and procedures.
• Implements the IDG nursing care plan for each patient.
• Provides nursing services, treatments and diagnostic and preventive procedures as assigned.
• Observes signs and symptoms and reports to the physician and RN reactions to treatments and changes in patient's condition.
• Teaches and counsels patient and family about nursing care needs.
• Evaluates with RN the effectiveness of nursing service to the patient and family.
• Maintains accurate records of observations, treatments and care.
• Participates in medical record audits as assigned.
• Attends staff meetings and IDG conferences and in-services.
• Takes on-call duty, nights, weekends and holidays as assigned.
• Submits schedule changes daily to Director/Manager of Patient Services.
• Participates in IDG conferences to discuss involvement of other health team members.
• Prepares clinical and progress notes.
• Assists physician and RN in performing specialized procedures.
• Prepares equipment and materials for treatments.
• Assists patient/family in learning self-care techniques.
