The role requires an active LPN license and a valid driver\'s license, with hospice experience preferred. Responsibilities include educating families and ensuring timely communication of changes in patient condition.

Qualifications

• LPN license required for practice.
• Hospice experience preferred.

Responsibilities

• Provide direct nursing care to hospice patients in home and facility settings.
• Assess patient needs and monitor symptoms, vital signs, and changes in condition.
• Administer medications, treatments, and interventions as ordered.
• Educate and support patients and families regarding symptom management and end-of-life care.
• Communicate changes and concerns to the RN and interdisciplinary care team.
