Job summary

Qualifications
- Graduate from an accredited school of nursing.
- Nurse Practitioner is an advanced practice RN with graduate-level education and national board certification.
- Current license as an NP to practice in the state.
- Experience: 2+ years as an NP preferred; active patient contact in last 3 years preferred.
Responsibilities
- Provide NP care to terminally ill hospice patients.
- Assist family understanding in home care and bereavement.
- Order, perform, supervise and interpret diagnostic tests.
- Develop and manage treatment plans and medications.
- Coordinate with IDG and hospice team for continuity of care.
- Teach and counsel patients and family; supervise nursing staff.
- Maintain patient records and follow HIPAA policies.
