Compassus is seeking a Hospice Licensed Practical Nurse (LPN/LVN) to deliver direct nursing care under the RN Case Manager, enabling patients to enjoy quality time with loved ones. You will educate families, collaborate with the care team, and advocate for comfort and dignity in a home-based hospice setting.

The role emphasizes compassionate, patient-centered care, with a focus on supporting families and improving quality of life while adhering to the Plan of Care.

Qualifications

• LVN/LPN licensure required or equivalent.
• Healthcare licensure appropriate for Missouri.

Responsibilities

• Provide direct, skilled nursing services in accordance with the Plan of Care under the direction of the RN Case Manager.
• Build trusting relationships with patients and families through compassionate care.
