Job summary

Bluegrass Care Navigators is seeking a Licensed Practical Nurse (LPN) to deliver compassionate nursing care to hospice patients in patients' homes, nursing facilities, or other living settings. You will follow the plan of care under a Registered Nurse, prioritizing patient comfort and quality of life at the end of life.

The role requires an LPN license, graduation from an approved program, and at least two years of nursing experience (hospice or long-term care preferred).

Qualifications

• Graduate of an accredited practical nurse program.
• Minimum of two years of nursing experience, hospice or long-term care preferred.
• Must be licensed to practice as an LPN and comply with professional nursing standards.
• Driver with a reliable vehicle in good working order and insured.

Responsibilities

• Provide skilled nursing care to hospice patients in their home or other settings.
• Work under the supervision of a Registered Nurse to implement the plan of care.
• Monitor patient condition, educate patients and caregivers, and document care in EMR.
