Qualifications

• Previous experience in hospice, home health, or healthcare intake/admissions preferred.
• Familiarity with hospice regulations, Medicare/Medicaid eligibility, and insurance verification.
• Excellent communication and interpersonal skills with a compassionate, patient-centered approach.
• Strong organizational skills with the ability to manage multiple referrals simultaneously.
• Proficiency with EMR systems and general computer applications.
• Ability to work effectively in a fast-paced, emotionally sensitive environment.
• High School Diploma or equivalent.

Responsibilities

• Manage the hospice intake process from initial referral through patient admission.
• Coordinate with physicians, hospitals, skilled nursing facilities, and care teams to gather clinical information.
• Communicate compassionately with patients, families, and caregivers to explain hospice services and eligibility.
• Verify insurance benefits, including Medicare, Medicaid, and private insurance for hospice coverage.
• Maintain accurate and timely documentation within the electronic medical records (EMR) system.
• Collaborate with clinical and administrative staff to ensure seamless care transitions.
• Track and follow up on pending referrals to maximize timely patient enrollment.
