Qualifications

• Demonstrated customer service skills with at least 1 year of experience.
• Proficiency in medical terminology acquired through a minimum of 1 year of experience.
• Advanced computer skills, with proficiency in navigating EHR systems and the Microsoft Suite.

Responsibilities

• Reviewing patient records to determine eligibility for hospice care.
• Communicating with healthcare providers, physicians, and other involved parties to gather necessary information.
• Interacting with patients and families to explain hospice services and answer questions.
